All students are expected to show respect for teachers, fellow students, personal property and the facilities of the Chinese School of CAACF, Inc. The Chinese School of CAACF, Inc. is a non-profit organization run by volunteers and donation is always encouraged.

I, the undersigned parent/legal guardian of _______________________________, a student registered in any of the programs/classes offered by the Chinese School, do hereby acknowledge and assume full responsibility for the safety of myself, my children, any member of my family and any visitors accompanying me, while attending the programs/classes offered by the Chinese School. I acknowledge that I will carefully listen to and follow all instructions and precautions presented by representatives of the Chinese School. I also agree to indemnify and hold the Chinese School, Its officers, directors, employees, agents and/or duly authorized representatives, successors and assigns (the ”Chinese School Entities”) harmless of and from any loss, liability, damage, cost of attorney fees incurred or caused by me or my children or by my participation or my child’s participation at the Chinese School, including without limitation any claims for personal injury or personal property damage which may be incurred by me, my children, any member of my family, and any visitors accompanying me at the Chinese School venue. I further agree to be fully responsible for any claims and damage caused by me, any member of my family and any visitors accompanying me, to the personal property, equipment, and physical structure of the Chinese School venue.

General Consent, Waiver, and Release:
I, the undersigned parent/legal guardian of _________________________________, a student registered in any of the programs/classes offered by the Chinese School of CAACF, Inc. (“Chinese School”), do hereby voluntarily consent, authorize, and grant permission to the Chinese School. Its officers, directors, agents, employees, and/or duty authorized representatives, successors and assigns (“Chinese School Entities”) to take photographs, recordings, or videos of me, and my child, any member of my family and any visitors accompanying me, while attending the programs/classes offered by the Chinese School and to record me and my child’s name, voice, picture and likeness and that of any member of my family and any visitors accompanying me, and reproduce my and my child’s the name, voice, picture and likeness and that of any member of my family and any visitors accompanying me, while attending the programs/classes offered by the Chinese School in connection with any advertising, publicity, promotion, marketing and/or other activities related to the Chinese School and no further consent shall be required for the publication, circulation, and dissemination of said photographs, recordings, or videos, or any duplication or facsimile thereof in any and all media now existing or hereafter created, for any purposes the Chinese School Entities may deem proper, including but not limited to the Chinese School website, reports, articles, and publications designed for educational, informational and promotional purposes.

I acknowledge that in granting the aforementioned permission. I do so freely and hereby relinquish and give to the Chinese School Entities all rights, title and interest that I, my child, any member of my family, and any visitors accompany me while attending the programs/classes offered by the Chinese School may have in the pictures, recordings, film, negatives, reproductions or copies (the “Photographs”) and further waive any right to compensation for the publication, distribution, or other use of said Photographs. This permission is given to the Chinese School Entities with the knowledge that it will be relied upon at substantial cost to the Chinese School Entities, and I agree on behalf of myself, my child, any member of my family, and any visitors accompanying me while attending the programs/classes offered by the Chinese School, not to assert any claim against the Chinese School Entities and to release the Chinese School, its officers, directors agents, employees, and duly authorized representatives from any and all claims of any nature whatsoever arising from the use of such Photographs.

I, the undersigned, agree to opt-in to the mailing list of the Chinese School and further consent, authorize and grant permission to the Chinese School, its officers, directors, agents, employees, and/or duly authorized representatives to release my, my spouse, and my child’s contact information, including name, address, phone number and email address provided on the student’s registration form, to the Chinese American Association of Central Florida (“CAACF”), the parent organization of the Chinese School, and agree to opt-in to the mailing list of CAACF for the purpose of receiving information relating to events and activities of both the Chinese School and CAACF.

I certify that I am over the age of eighteen and I declare and represent that no promise, inducement or agreement not expressly made in this Liability and Responsibility agreement has been made to me and that the terms of this Liability and Responsibility Agreement are contractual and not a mere recital. I have read the foregoing terms and conditions of this Liability and Release Agreement terms and conditions of this General Consent, Waiver and Release and fully understand it.
